We request that the name of the above-named child be registered as a prospective pupil. A cheque for non-returnable registration fee of QAR 500, made payable to RGS Guildford, is enclosed. We understand that the School’s acceptance of this registration form and fee does not constitute an offer of a place. We understand that the standard terms and conditions of the School will undergo reasonable changes from time to time as circumstances demand and will apply in all our dealings with the School.

We understand that the School (through the Principal, as the person responsible) may obtain, process and hold personal information about our child, including sensitive information such as medical details. We consent to this for the purposes of assessment for entry and, if a place is offered, in order to safeguard and promote the welfare of our child. We certify that our child has not been dismissed, nor his removal required, from any school on account of misconduct.
